Possible bad microSD card?

With the purchase of my touch, i bought a 16gb sd card from Verizon and have been having some troubles with it.  Previously, i just noticed and irritating delay between switching songs that were on the card, but now it is worse and the card keeps 'disappearing' from my phone.  The pictures loaded on it would disappear to the phone and when i go to the music player, no songs are added.  To fix this, i have to restart the phone, but then all of the songs have to be reloaded which takes about 10-15 min :/.  I'm starting to wonder whether or not i have gotten a bad SD card or if mine is just dieing.  Any thoughts?
